Some item names are changed. Maps are exactly the same, as far as I can tell. Non npc fixed encounters will respawn when you go to Llylgamyn (the Nether Demon, Dark Lord, and Arch Devil, for example). The snes version has great monster art.
There's mediocre music and maze graphics. You can turn the music off and switch to the original wire-frame mazes in an options menu.
The US version has a lot of censorship. Most obviously, female enemies don't reveal as much. Succubi are nude in the Japanese version; their US counterparts were redrawn and completely covered. Every variation of the word "death" was replaced by "doom," even when this messes up rhymes. Devils are Fiends in the US version. Blood was taken out of some monster art (Scarecrows, Berserkers).
