""Crushing" defeat!"

I was playing around in the Lower Monastery (started a new game with a Ninja and Valkyrie, characters I've never played before (Vi not withstanding)) and was caught in a battle with some slimes. Went behind a barrel to get them to come to me and suddenly I see sky. I turn and look out and it says, "Your party has been crushed". I walked right through the wall.

Kicky!

Go the bridge you operate (in the monastery) and raise it while not on it. Then inch a bit closer and lower it ... apparently our stalwart adventurers aren't smart enough to move there toes, out of the way of a slow moving bridge

this is why I can't understand anyone playing Ironman. It's WAY too easy to get accidently crushed in this game.

(I didn't realize my inadvertant joke last time, couldn't pass it up)

... but once you know where these little accidents can occur, it is (fairly) easy to avoid them. I think there is still an issue if you get a skill increase (like someone identifies a Rattkin Goon and their mythology goes up a point) while riding on the Trynton elevator, in the Rattkin tree area. The issue being that you fall through the floor of the elevator. You used to be able to do that just by spinning in circle, or panning your view, while riding them.

See, you just have to know what to avoid ... but I still wouldn't play Iron Man

LLevram is too honest to mention this, so I will.

Open the savegame folder and copy your Ironman game to the desktop. Then start the game. If something "too" bad happens, exit then copy/paste the desktop version back to the savegame folder and restart the game. You can recopy to the desktop after an autosave too.
