It seems that game somehow deletes one of the items in case of the name conflict such as:
I got the Gold key from the Crypt
Kol gave me the key for the Kerah statue in the Serpent Temple
now I am opening Kerah and kaboom Kol's key becomes Gold key and Gold key from Crypt is gone

Same happened with script which I have found in the Serpent Temple in the room above the library this script contained some sort of a puzzle but when I have got encrypted letter to the dragon this script disapeared so I do not even know what was written on it....

I have read quite a few posts on the other boards about people losing items when they swap characters in and out of there party ... perhaps you lost it there, doing that?

I have only one character in the party I have never swapped it...
